You can use this form to submit a request for a repeat prescription. You will need a copy of your repeat prescription slip, or an exact description of how you take your medicine, so that there is no confusion as to what you are ordering.
Requests will be picked up once daily at 9am and will then need to be processed.
Please then allow 2 working days for your request to be processed.
Please note, that for repeat items we will normally issue a maximum of a 2 months' supply at a time in order to minimise waste, which can be a huge cost to the NHS. The only exception to this is the contraceptive pill (where it is recommended to give a 3 or 6 month supply) or as a temporary measure in exceptional circumstances.
If there is anything unusual about your request - such as you are requesting it early due to a holiday, or requesting a new medication, or a different quantity to usual - then it will help us greatly if you could add a comment.
You can have your prescription left at the surgery or delivered to a local chemist.
Please stick to the same choice whenever possible as changing frequently is likely to lead to errors.
